Cornerstone Support provides individual, person-centred care and support plans, which identify the desired outcomes and details specific actions needed to achieve them. Our services are tailored for those with learning difficulties, autism and who are homeless.

Our service operates with flexibility, believing there is no such thing as a ‘one size fits all’ approach. We adapt our service to meet individual needs, desires, risks, and experience so we can best support individuals take their next step to independence.

Who do we support?

We support adults (18+) who have learning difficulties, autism, brain injuries and mental health challenges. Through working together on care plans, levels of support and adaptations needed, we enable each person to live the life they want.

We work collaboratively with community partners, such social services agencies, and mental health professionals to ensure those in our care find the appropriate support and services needed to live independently.
